· If you have the actual certificate, you will use a form called an irrevocable stock or bond power that can be completed and signed and have delivered to Larry Burgess along with the certificate. Do not sign the back of the certificate. The shares will be deposited to the CAP account. The value of your contribution will be based upon the value of the stock or bond on the day it posts to the CAP account. Contact Larry Burgess at the above phone number to obtain an irrevocable stock or bond power. · Or, if the stock is held in a brokerage account, you can request the other brokerage firm to transfer the stock to the CAP account directly. You will need to provide them with a written letter of instruction indicating you want to make a gift of the stock (include the CUSIP number of the stock and the number of shares to transfer) and request that the shares be transferred directly to the Exchange Club Center for Prevention of Child Abuse, Inc. account #3MD-328307, c/o Pershing, LLC DTC #0443. · Please notify the
