Volunteer Applications Instructions for completing the volunteer application: 1. Download Families First Volunteer Application. 2. Complete application. 3. Have Central Registery Check Consent notarized. 4. Return the completed application with notarized form to the CAP Center. 5. Provide your three references with the reference form at the end of the application. 6. Have three references fill out the form and return to the CAP Center. Upon receipt of your completed application, you will be contacted by the CAP Center. If you are a good match for one of our programs, we will schedule an interview with you to discuss the program and the volunteer opportunity in more depth.
